Reviewing and reflecting on the findings of coronial inquests can allow us to learn from past failures in order to help prevent avoidable deaths. In this lecture, associate professor Linda Starr reviews a coroners case through a quality improvement lens, identifying opportunities to improve patient outcomes through better communication and documentation - particularly where clinical deterioration is present.
