Holistic Care Planning
Holistic care planning is a cyclic process that begins with accurate and comprehensive assessment of the lifestyle and needs of the person receiving care. It is a systematic attempt to provide a framework for staff and family caregivers to follow in the daily delivery of care, and an attempt to put in place monitoring tools to evaluate whether the care strategies implemented are successful. Care planning should be dynamic with new strategies constantly being devised and current plans evaluated. The process of assessment can be thought of as data collection necessary for understanding the unique lifestyle of the care recipient. The more data gathered the more likely it is that the approach to care will have satisfactory outcomes.
